Memory Nurture offers a range of services in the home, supported accommodation and aged care facilities for people with dementia/memory loss and support to their families.
We can provide:
Memory Nurture can provide services to clients receiving a home care package. We work with several organisations for example Amana Living, Silver Chain, and Baptist Care. We can negotiate brokerage agreements with any aged care providers. If you would like more information, please contact us.
We are a registered NDIS provider for therapeutic supports, aids and equipment. We can complete NDIS assessments including Functional Capacity Assessments, Home Assessments and Community Access Assessments. Please see our NDIS page for more information. here
COPE program – in home support for families caring for a loved one with dementia here
For Aged Care Facilities –
Memory Nurture can provide Mental Health support through the Medicare scheme Better Access. Our qualified Mental Health OT can provide individual or group sessions to eligible clients. Through this program, eligible clients can receive up to 10 sessions a year. If you have a client who would benefit from the program to assist with anxiety, depression, or other mental health issue, please give us a call.
